01

Parties & Effective Date

This Service Agreement (“Agreement”) is entered into between Systems of Care Doc (“we,” “us,” “our”) and the individual or family signing below (“Client,” “you”). It takes effect on the date you electronically sign it and remains in effect until terminated by either party under Section 13.

02

Nature of Services

We provide non-clinical medical advocacy, care coordination, and liaison services. Our role is to help you understand, organize, and navigate the healthcare system — not to replace it. Services may include:

  • Direct physician consultations for second opinions and medical guidance
  • Full-service medical liaison and care coordination
  • Medical record review, organization, and summarization
  • Appointment, specialist, and referral coordination
  • Communication support between you and your care team
03

Scope of Services & Medical Disclaimer

Dr. Tania Mason-Eastmond is a licensed physician; however, the advocacy and coordination services described here are distinct from an ongoing treating physician–patient relationship. Signing this Agreement does not create a physician–patient relationship unless we separately agree to that in writing.

We do not diagnose, prescribe, or provide emergency care under this Agreement. If you are experiencing a medical emergency, call 911 or go to your nearest emergency department immediately.

04

Scheduling & Appointments

  • Consultations are scheduled at mutually agreed times through our online scheduling system.
  • Please join promptly. Arriving more than 15 minutes late may require shortening or rescheduling your session.
  • Circumstances change — please give at least 24 hours' notice for cancellations or rescheduling.
  • Repeated no-shows or late cancellations may require a pre-payment deposit for future bookings.
05

Fees & Payment

  • Fees are transparent and communicated to you before you book.
  • Your initial 30-minute consultation is complimentary.
  • Payment is due at the time of service unless we agree to other arrangements in writing.
  • Payments are processed securely through Stripe. We never store full card details on our servers.
  • We do not bill insurance for advocacy services, but we can provide documentation you may submit to your insurer for possible reimbursement.

07

Confidentiality & Privacy

All information you share with us — before, during, or after a session — is treated as confidential. We comply with applicable privacy laws and maintain HIPAA-conscious safeguards for protected health information. We do not sell, rent, or trade your information. It is shared only with your written authorization, with providers involved in coordinating your care and with your consent, or when required by law. Full details are in our Privacy Policy and HIPAA Notice.

08

Client Responsibilities

You agree to:

  • Provide accurate, complete, and current information about your medical situation.
  • Obtain and share relevant medical records and authorizations as needed.
  • Keep us informed of changes to your health status or contact information.
  • Treat our staff and providers with respect and professionalism.
  • Follow through on agreed action items between sessions.

09

Communication & Records

Routine communication may take place by email, phone, or secure messaging. Standard email and text messages are not fully secure channels, so we avoid sending detailed clinical information through unencrypted channels. You may ask us to use a specific method or address for communication at any time.

10

Independent Advocacy Relationship

You retain full control over your healthcare decisions. Our recommendations are advisory. You are free to accept, decline, or seek alternative guidance at any time, and we encourage you to maintain your relationships with your treating providers.

11

Intellectual Property

All written materials, summaries, templates, and educational content we provide are the property of Systems of Care Doc and are for your personal use. They may not be reproduced, distributed, or used commercially without our prior written consent.

12

Limitation of Liability

To the fullest extent permitted by law, Systems of Care Doc, its owner, and its staff shall not be liable for indirect, incidental, consequential, or special damages arising from the use of our services, except as explicitly provided in this Agreement. Our total liability is limited to the fees paid for the specific service giving rise to the claim. Nothing in this Agreement limits rights you may have under applicable law.

13

Termination of Services

Either party may terminate this Agreement with reasonable written notice. Upon termination, we will provide a summary of services rendered and any applicable refunds consistent with our cancellation policy. We may decline or discontinue services if we determine they are not appropriate for the situation or if the working relationship has become untenable.

14

Changes to This Agreement

We may update this Agreement from time to time. The current version will always be available on this page, and material changes will be communicated to active clients. Continued use of our services after changes take effect constitutes acceptance of the revised terms.

15

Governing Law & Dispute Resolution

This Agreement is governed by the laws of the State of New Jersey, without regard to conflict-of-law provisions. If you have a concern, we ask that you raise it with us directly first so we can work toward a resolution in good faith.
